प्रस्तावना

मेहरानपुर की मनीषा एक software engineer हैं। उन्होंने braces से दाँत सीधे करवाए, whitening करवाई — हर तरफ से smile perfect थी। लेकिन एक बात थी — उनके मसूड़े का shape ऊबड़-खाबड़ था। एक तरफ का मसूड़ा नीचे था, दूसरी तरफ ऊपर। Orthodontist ने कहा — “यह gingivoplasty से ठीक हो जाएगा।”

“Gingivoplasty” — यह शब्द internet पर ढूँढने पर मनीषा को बहुत कम जानकारी मिली। लेकिन यह एक बेहद सरल और प्रभावी cosmetic procedure है जो मसूड़े के आकार को सुंदर और symmetrical बनाती है।

Gingivoplasty और Gingivectomy में बहुत लोग confusion करते हैं। इस लेख में दोनों का फर्क और gingivoplasty की पूरी जानकारी साझा करते हैं।

Gingivoplasty क्या है?

Gingivoplasty मसूड़े के आकार और contour को reshape करने की cosmetic dental procedure है।

Gingivectomy से फर्क: Gingivectomy में excess tissue को हटाया जाता है (मात्रा कम की जाती है)। Gingivoplasty में tissue को reshape और recontour किया जाता है — ज़रूरी नहीं कि बहुत ज़्यादा tissue हटाई जाए।

एक analogy से समझें: Gingivectomy एक haircut की तरह है (बहुत लंबे बाल छोटे करना)। Gingivoplasty एक hairstyling की तरह है (बालों को सुंदर shape देना)।

Gingivoplasty में:

• मसूड़े की ऊँचाई को एक समान किया जाता है
• Gum line को smooth और curved बनाया जाता है
• अनियमित, flat या पुराने surgery के बाद distorted gum को ठीक किया जाता है
• Gum के contour को natural और healthy appearance दी जाती है

यह अक्सर gingivectomy के साथ-साथ की जाती है। इसे gum contouring या gum reshaping भी कहते हैं।

Scalpel, electrosurgery unit या diode laser से की जाती है। Laser gingivoplasty आजकल सबसे preferred है।

मरीज़ इस इलाज से क्यों डरते हैं?

• “मसूड़े reshape होंगे — क्या natural दिखेंगे?” — एक skilled periodontist या cosmetic dentist digital planning से पहले आपको expected result दिखाते हैं। Result absolutely natural होता है — जैसे naturally perfectly shaped gums।
• “क्या बहुत दर्द होगा?” — Local anesthesia से पूरी तरह सुन्न। Laser gingivoplasty में pain न के बराबर होती है procedure के दौरान और बाद में भी।
• “क्या sensitivity बढ़ेगी?” — थोड़े हफ्तों की sensitivity possible है। Sensitive toothpaste से यह manage होती है।
• “क्या एक बार में सब होगा?” — हाँ, आमतौर पर एक sitting में। कुछ complex cases में दो sessions हो सकते हैं।
• “क्या यह permanent है?” — हाँ। एक बार reshape हुए मसूड़े वापस पुराने shape में नहीं आते, जब तक की कोई नई बीमारी या overgrowth न हो।

• “यह कहाँ तक ठीक होगा — सब दाँतों के मसूड़े?” — यह आपकी ज़रूरत पर निर्भर है। 4 front teeth, 6 front teeth या full arch — plan अनुसार।
• “क्या orthodontic treatment के बाद ज़रूरी है?” — हमेशा नहीं। लेकिन अगर braces के बाद gum overgrowth हो या gum line asymmetric हो तो gingivoplasty उचित है।

Gum reshaping का cosmetic चित्रण

यह इलाज कब करवाना चाहिए?

1.  Gum line असमान हो — एक तरफ ऊँची, दूसरी तरफ नीची — front teeth के ऊपर gum level uneven दिखे।

2.  Orthodontic treatment के बाद gum irregularity — braces हटाने के बाद कुछ teeth पर gum overgrowth हो।

3.  Gingivectomy के बाद finishing — tissue हटाने के बाद edge को smooth और natural curve देने के लिए।

4.  Smile Design का हिस्सा — veneers, crowns या whitening के साथ complete smile makeover में gum contouring।

5.  दाँत proportionally छोटे दिखते हों — excess gum दाँत को छोटा दिखाए।

6.  Gum का shape flat या square हो — naturally gum scalloped (curved) होती है। Flat gum line unnatural लगती है।

7.  Gum recession के बाद contouring — जहाँ gum नीचे गई हो उसके आसपास contouring।

इलाज की प्रक्रिया कैसे होती है?

करने वाला: Periodontist या Cosmetic Dentist

1.  Consultation और Digital Planning — dentist आपकी smile का analysis करते हैं। Computer software या digital photographs से expected result plan और दिखाया जाता है।

1.  Shade और Symmetry Analysis — कौन से दाँत के ऊपर कितना tissue reshape करना है — यह millimeter level पर plan होता है।

1.  Local Anesthesia — उस क्षेत्र को syringe से सुन्न किया जाता है।

1.  Marking — periodontal probe से हर दाँत के ऊपर gum का desired level mark किया जाता है।

1.  Reshaping — scalpel या laser से gum को mark के अनुसार reshape किया जाता है। Scalloped, natural curve दिया जाता है।

1.  Smoothening और Polishing — edges को smooth किया जाता है ताकि final result natural दिखे।

1.  Irrigation — chlorhexidine से क्षेत्र साफ किया जाता है।

1.  Periodontal dressing (यदि ज़रूरी) — कुछ cases में protective dressing लगाई जाती है।

1.  Final Check — smile को mirror में दिखाया जाता है, feedback लिया जाता है।

इलाज के बाद देखभाल (Post-treatment Care)

1.  Soft Diet पहले 1-2 सप्ताह — नरम, ठंडा खाना खाएं। Hot, spicy और crunchy टालें।

2.  Chlorhexidine mouthwash — दिन में दो बार prescribed mouthwash।

3.  Soft Toothbrush — healing के दौरान extra-soft toothbrush use करें।

4.  Surgical area पर brush न करें — पहले सप्ताह उस tissue को directly brush न करें।

5.  Prescribed दवाएं लें — anti-inflammatory और antibiotic (यदि दी हो)।

6.  Smoking बिल्कुल बंद करें — healing को बाधित करती है।

7.  Salt water warm rinse — दिन में 2-3 बार।

8.  Follow-up — 1 सप्ताह बाद healing check, dressing removal (यदि लगाया हो)।

9.  Sensitive toothpaste — अगर sensitivity हो तो fluoride-based sensitive toothpaste use करें।

10.  Avoid Physical Exertion — पहले 24 घंटे gym, sports, heavy lifting से बचें।

प्रश्न: Gingivoplasty और Gingivectomy एक साथ होती हैं?

उत्तर: अक्सर हाँ — जब excess tissue को हटाया जाता है (gingivectomy) तो उस edge को smooth और naturally curved बनाना ज़रूरी होता है (gingivoplasty)। दोनों एक ही sitting में होती हैं। लेकिन gingivoplasty अकेले भी की जा सकती है — जब tissue की मात्रा ठीक हो पर shape distorted हो।

प्रश्न: क्या gingivoplasty के बाद veneer और crown का result बेहतर होता है?

उत्तर: बिल्कुल। Veneers या crowns की planning में gum level एक critical factor है। अगर gum asymmetric है तो veneer लगाने के बाद भी smile uneven दिखेगी। Gingivoplasty के बाद gum level plan करके crowns और veneers लगाने से result dramatically बेहतर होता है।

प्रश्न: Gingivoplasty के लिए सबसे अच्छी उम्र क्या है?

उत्तर: आमतौर पर adult teeth (permanent teeth) पूरी तरह erupt होने के बाद — यानी 18 वर्ष के बाद। इससे पहले gum और teeth अभी develop हो रहे होते हैं। जो teens orthodontic treatment करवा रहे हों उनमें braces complete होने के बाद gingivoplasty की planning होती है।

प्रश्न: क्या gingivoplasty से gum recession हो सकती है?

उत्तर: अगर सही मात्रा में tissue हटाई जाए और underlying bone structure adequate हो तो recession का risk नहीं। Over-aggressive reshaping से recession possible है — इसीलिए skilled और conservative approach वाले periodontist को choose करें। Digital planning से यह risk minimize होता है।

प्रश्न: क्या gingivoplasty के बाद sensitivity permanent रहती है?

उत्तर: नहीं, sensitivity temporary है। 4-8 सप्ताह में अधिकांश मरीज़ों की sensitivity कम या समाप्त हो जाती है। इस दौरान sensitive toothpaste (potassium nitrate या stannous fluoride based) और fluoride gel से बहुत आराम मिलता है। Persistent sensitivity हो तो dentist से मिलें।

प्रश्न: गम contouring (gingivoplasty) के बाद mुस्कान कैसी लगती है?

उत्तर: मरीज़ अक्सर बताते हैं कि result उनकी उम्मीद से बेहतर होता है। Gums की symmetrical line और properly proportioned teeth मिलकर एक naturally beautiful smile बनाते हैं। Self-confidence में भी बड़ा फर्क आता है — खुलकर हँसना अच्छा लगता है।
